PRIMARY FUNCTION OF POSITION
The Youth Health Promotion Officer’s position encompasses a broad range
of services specifically targeting young people in the Blacktown Local
Government Area. Included in this role is a commitment to working with
schools, services and with young people in the broader community to address
relevant health issues. This will be achieved by working collaboratively with
a range of government and non-government agencies to provide support to young
people in the local community and apply for funding to run projects and
programs that meet identified needs including addressing barriers faced by
young people accessing local services such as WAAT. The Youth Health
Promotion Officer’s role is an important position within the Western
Area Adolescent Team in providing a range of multidisciplinary Youth Health
services including Drop In, On Call and Outreach.
SELECTION CRITERIA
Essential:
1. Relevant qualifications related to working in an Health Promotion position
such as social science or health
2. Previous experience working with disadvantaged young people and
demonstrated understanding of relevant issues
3. Demonstrated experience in planning, delivering and evaluating health
promotion activities and/or community development projects to meet the needs
of an identified target group
4. Demonstrated effective networking/partnership development skills
5. Current driving license with proven good driving record
6. Demonstrated experience working with a range of computer software packages
including Word
Desirable:
1. Experience working in a community based setting.
2. Experience working with young people from diverse cultural backgrounds.

STATEMENT
Community Health services are provided from seven major community health
centres, two youth health services, thirty three service specific sites such
as early childhood and aged respite centres and over one hundred community
nursing clinics in schools. Primary Care and Community Health Network employs
approximately 520 full time equivalent staff who work in multidisciplinary
teams, each with a team leader or service manager.
Community Health Services deliver a range of multidisciplinary health
services utilising primary health care approaches and current best practice
to maintain and improve the health of local communities. A combination of
primary and secondary level services are delivered across the five local
government areas (Auburn, Baulkham Hills, Parramatta and Holroyd) in
collaboration with individuals, communities and service agencies. All service
delivery and activities are underpinned by agreed values of equity,
participation, integrity, respect, quality and trust.
SERVICE INFORMATION
Western Area Adolescent Team (WAAT) is a youth health service working with
young people aged 12-20 years who live, study or work in the Blacktown Local
Government Area. WAAT adopts a holistic approach to health and is committed
to providing appropriate services which reflect the diversity of the youth
population. It is WAAT’s belief that access to accurate information,
education and support enables young people to increase their health options
and improve their health outcomes.
This service is located at Mt Druitt Community Health Centre and has a full
time equivalent of 12 staff. WAAT provides the following services:
• On Call includes the provision of information, support, advocacy,
brief intervention and referral to the WAAT counselling service or a more
appropriate service.
• Counselling is available and accessible to all young people via the
On Call system. Referrals can come from young people, families, school staff
or other agencies. A range of appropriate counselling models are used that
may best meet the needs of young people. The service provides both generalist
& Alcohol and Other Drugs Counselling.
• Key Work is a case work service provided to WAAT clients who have a
range of high support needs.
• Health Promotion strategies are used to meet the physical, emotional
and mental health needs of young people. These include health education,
small group work and community projects which may include healthy relationships,
alcohol and other drugs, mental and emotional health, sexual health and self
esteem.
• Drop-In service enables young people to engage with the service in an
informal manner through group activities, basic living skills and nutrition
programs.
• HIV/ Hep C Prevention Service offers clean safe injecting equipment,
referral, information, advocacy, brief intervention, safe sex advice,
resources and Outreach using a Harm Minimisation approach.
• Nursing Services provide a variety of clinical and psychosocial
support services to young people. This may include home visiting to young
parents based on a Families First philosophy. Issues addressed will include
general youth health issues, immunisation, sexual health, contraception,
pregnancy and termination advice.
• Outreach is a service which provides information, advocacy, support
and brief interventions to young people off-site across the LGA.

PERFORMANCE MANAGEMENT
A Performance Management Plan based on this statement of duties will be
developed in conjunction with the Services Manager and achievements will be
reviewed on an annual basis. Work performance will be managed on a continual
basis with the Service Manager. The initial performance interview will occur
three months following appointment and at regular intervals on an ongoing
basis. Performance management will focus on identified key responsibility
areas of the position.
DETAILED POSITION DUTIES
1. HEALTHIER PEOPLE.
1.1 Health Promotion and Community Development
Participate in the design, implementation and evaluation of preventative
health promotions activities and community programs within schools, refuges
and youth centres.
Incorporate the principles and strategies of health promotion and education
that address youth specific issues into group facilitation and team
initiatives.
Identify opportunities for involvement in and the funding of relevant
community networks and projects.
Develop, implement and evaluate appropriate resources for young people, their
families and other service providers, particularly those relating to access
issues.
Advocate on behalf of and in conjunction with the target group in regard to
issues relating to youth health including access to youth focused services.
Provide consultation and support to team members, health services and other
youth services about the needs of young people as required.
Represent the service on appropriate management committee/advisory
councils/working groups as required.
1.2 Early Identification & Intervention
Utilise available resources for early identification of and intervention in
potential health issues for young people.
Ensure that early identification approaches are implemented in a timely,
accurate and comprehensive manner and conform with current best practice.
Provide prompt and appropriate interventions to young people and their
families who have identified health problems to maintain or improve their
health.
Identify and address the health needs of young people providing opportunities
for early identification and intervention, health education and health
promotion.
Identify and address the barriers faced by young people in the Blacktown LGA
accessing youth focused services including WAAT.
1.3 Clinical Practice
Provide appropriate general health, sexual health, pregnancy and harm
minimisation information to local young people as required.
Participate in the Worker On-Call system to provide assessment, intake and
brief intervention, consultation and referrals to young people and their
families who present to the service.
Assist with the design and implementation of harm minimisation programs
including the provision of the needle and syringe program.
Participate in and assist with service activities including Drop In and
Outreach on a regular basis.
Provide behavioural interventions in conjunction with other staff to manage
violent and aggressive situations, substance misuse and other situations
requiring intervention.
Maintain accurate and up-to-date client records in accordance with Community
Health policy.
Attend and participate in weekly team meetings and present activities within
the team as required.
Participate in clinical peer support opportunities.
Provide consultation to staff from other disciplines across the Centre.
Participate in the performance management process, including creation and
implementation of a personal development plan.
Undertake clinical supervision as per Community Health Clinical Supervision
policy.
2. FAIRER ACCESS.
2.1 Primary Health Care
Incorporate the principles of primary health care into day to day work
practice.
Ensure the practice and philosophy of primary health care is demonstrated in
work outcomes and that these outcomes are accessible and appropriate to the
identified needs of the local communities and consistent with the
expectations of key service partners.
Develop and implement strategies to ensure work outcomes are culturally
sensitive to the needs of the local community.
Implement the Community Health - Community Participation Model to ensure
effective consumer involvement in the planning, delivery and evaluation of
clinical services.
2.2 Equity & Access
Ensure the service promotes equity and undertakes initiatives designed to reduce
health inequities in the local community and maximise service access.
Work with young people, health staff and the community to identify young
peoples health needs and to develop appropriate strategies to deal with them.
Provide consultancy on youth health issues to school personnel, health
workers and other agency workers as appropriate.
Participate in the development, implementation and review of the service
Ethnic Affairs Priority Statement (EAPS) Plan as requested.
3. QUALITY HEALTH CARE
3.1 Continuous quality improvement
Attend regular supervision.
Participate in staff development and in-service training as appropriate.
Attend seminars, lectures, conferences and supervision as required for
professional development.

Ensure all cash handling activities are undertaken according to SWAHS Cash

Utilise available IT resource to support health promotion practices.
Report to the Service Manager relevant IT requirements, issues and training
needs.

development activities.
