Has anyone developed a youth
friendly Disclosure of Personal Information Consent Form and also gained
approval for it from the Privacy Commission. In particular I am
seeking a ‘blanket’ type disclosure where a young person consents
to transfer of communications occurring with agencies and organisations
regarding the support of their education, training and employment requirements
without each time having to have them sign off on same – ie forwarding of resumes to employers, referral to courses
etc – Express consent would be obtained in sensitive circumstances ie – referral to counseling etc.
If anyone has one, or has some advice in making this process
easier, I would be grateful of same
